Rolling Application: Powered Wheelchair Handcycle Attachment or Adaptive E-Bikes
Introduction
This Rolling Application is for adults with chronic or permanent disabilities who are looking to buy a powered handcycle wheelchair attachment or adaptive e-bike modified to their needs. For the Rolling Application, Vouchers will be distributed in order of Application after eligibility has been verified and bike fitting is completed.

Vouchers issued through this application can be used only for the following:
Adaptive E-Bikes are specialized bicycles that meet the physical needs or abilities of the operator or a passenger, who has a chronic or permanent disability.
Powered Handcycle Wheelchair Attachments are devices designed to convert manual wheelchairs into electric-powered handcycles.
It should be noted that vouchers received through this application can only be redeemed at specific retailers who are approved to sell adaptive equipment and specialize in fitting such bikes.

To qualify for an adaptive e-bike, the applicant must have a chronic disabling condition that significantly impairs their ability to ride a standard e-bike. Examples of such conditions include a person who uses a wheelchair, walker or crutches; relies on oxygen; has conditions impacting their ability to balance while standing, walking, or riding a bike; or in some other way is unable to use and enjoy a standard or cargo e-bike. After completing the rolling application, eligible applicants will be required to attend one of the bike fitting sessions with experts to get an appropriately modified adaptive e-bike. Following the fitting session, the applicant will receive the voucher.
On the next page you will be asked to select your application type. There are two options:
Adult with Permanent Disability: chose this if your income is above 40% of the AMI.
Income-eligible Adult with Permanent Disability chose this if your income is at or below 40% of the AMI.
